On the point about the engine incident having "gone quiet", we would respond that having communicated the basic circumstances of the incident publicly within 24 hours, and submitted the required Mandatory Occurrence report to the CAA, the Trust then started a formal investigation as expected of an organisation with strict safety management policies, under the leadership of a Board member.
This resulted in an Investigation Report containing recommendations that was sent to the Board and the Executive Team. The recommendations have been accepted and implemented, and recently a closure report was sent by the Executive Team to the Board.
A summary of these reports will be published once the Board has responded, taking into account the implications of our "No Blame" policy.
